Follow Up Care
While you were at Remuda, your recovery was our goal. Months, even years after you have returned home, that remains our objective. That’s why we have our Follow Up Coordinator contact you at regular intervals: approximately 10 days, 30 days, 3 months, 6 months, 1 year and 2 years after discharge. These calls are designed to help you in areas such as your aftercare plan. We also want to update you on various enrichment programs for Remuda alumni, which are created from input provided by former patients like you.

Aftercare
As you recall, you participated in Relapse Prevention Groups that helped establish your aftercare plan. Although we provided you with a list of resources, this was not intended to replace any current relationships you may have with your home therapy team. Indeed, if you have a therapist or other professionals at home who are appropriate for ongoing care, we encourage you to continue outpatient treatment with them. Yet, if you ever need additional/different resources in your area or another area due to relocation, please contact us for a list of professionals. These names have been compiled from other referring professionals, our patients, their families, and other therapy organizations. We use every avenue possible to provide you with a thorough list of options to help build a complete aftercare team.

Quality Care
When you established an aftercare team, we provided them with information about Remuda and your treatment here. We forwarded your medical records to those professionals listed on your aftercare plan to ease the transition to outpatient treatment. If someone on your team still needs your medical records or has questions about your treatment, please have them contact our Quality Care Coordinator.
